Psyllium
Psyllium, obtained from the plant Plantago ovata, is a predominantly soluble fiber with pronounced bulking properties. It contains 15-25% fermentable components and its main carbohydrates are arabinose and xylose, which combine to form arabinoxylans, a complex matrix that may contribute to the health and diversity of the gut microbiome (Marlett & Fischer, 2002; Al-Assaf et al., 2003; Zannini et al., 2022).
In the intestine, psyllium binds to large amounts of water and forms a gel that increases the volume and weight of feces and improves their consistency (Marlett, Kajs & Fischer, 2000; Van Der Schoot et al., 2022; Van Der Schoot et al., 2024). This is why it is preferred by many clinicians for the treatment of chronic constipation, with an additional advantage of its relatively gentler action compared to stimulant laxatives (Siegel & Di Palma, 2005).
Comparisons with similar plant fibers have shown that its intake leads to less bloating, abdominal pain, gas, and nausea (Sheer & Alois, 2024). Another interesting property is that it promotes the formation of the short-chain fatty acid butyrate in the intestine, which may have a preventive effect on the growth of pathogenic microorganisms and improve the absorption of some minerals from food (Bourquin et al., 1993; Fernández-Bañares et al., 1999; Timm et al., 2010).
California buckthorn
California buckthorn (Rhamnus purshiana) is a plant native to the Pacific coast of North America, and its bark has been dried and used for medicinal purposes since long before the continent was discovered by European explorers. Because of this use, the product is also known as cascara sagrada, or "sacred bark" in Spanish.
The laxative properties of California buckthorn bark have been known in Europe since at least 1880 (Madaus, 1938) and the preparation has a long history of use in patients with acute and chronic constipation (Covington, et al., 1996). The well-established historical use of cascara sagrada has led to its inclusion in authoritative monographs of the German Kommission E (Kommission E, 1993), the European Scientific Cooperative on Phytotherapy (ESCOP, 1999) and the World Health Organization (WHO, 2002).
Based on evidence of long-standing use with demonstrated efficacy and safety, in 2019 the Committee for Herbal Medicinal Products of the European Medicines Agency (HMPC) accepted the well-established use of sea buckthorn bark preparations as a stimulant laxative (EMA/HMPC/909434/2019).
Get off
Mallow (Malva sylvestris) is an annual herbaceous plant native to Europe, North Africa, and Asia. It was used in ancient Greece and Rome for its pronounced emollient, antispasmodic, and laxative properties, and in Central Anatolia it is widely used in cooking (Barros et al., 2010).
The laxative effect of mallow is probably due to the high concentration of mucilages in the leaves and bark of the plant. These so-called mucilages are extremely rich in soluble fiber and polysaccharides and are particularly effective in binding available water, demonstrating thickening, adhesive and softening properties (Karawya et al., 1971).
Once in the intestines, these substances transform into a viscous gel-like mass that stimulates peristalsis. Unlike some chemical laxatives, they do not irritate the stomach and intestines, as they form a thin protective film on their surface.
Of additional interest is that mallow extract is rich in phenolic compounds that counteract oxidative stress – a mechanism likely involved in the pathogenesis of constipation (Halliwell, Gutteridge & Cross, 1992; Jabri et al., 2017).
Clinical trials of mallow have shown that taking a flower extract after meals significantly increases defecation frequency and reduces stool hardness in patients with functional constipation (Elsagh et al., 2015).
Probiotics
Probiotics are bacteria that help metabolize food in the gastrointestinal tract and contribute to intestinal health, and the scientific evidence for their use in combating constipation is promising.
The most extensive meta-analysis of clinical trials in adults with functional constipation showed that probiotic intake increased the number of weekly bowel movements compared to placebo (Dimidi et al., 2014).
Of particular interest in the literature is the species Lactobacillus casei, which has a long history of use for the relief of constipation among diverse populations (Koebnick et al., 2003; Sakai et al., 2011; Mazlyn et al., 2013; Aoki et al., 2014).
The regulatory effect of this lactobacillus has also been directly tested in a number of placebo-controlled clinical studies, which confirm that its intake successfully reduces the transit time of intestinal contents, improves defecation and relieves symptoms of constipation (Koebnick et al., 2003; Krammer et al., 2011; Mazlyn et al., 2013).
It is suggested that the observed effects may be at least partly related to the property of Lactobacillus casei to modulate the intestinal microbiota and secrete short-chain fatty acids, which change the physical characteristics of feces and accelerate its movement through the intestinal tract (Chen et al., 2019).
Another probiotic with therapeutic potential is Bacillus subtilis, which reliably reduces gastrointestinal discomfort and normalizes stool consistency, reducing constipation-diarrhea alternation in healthy patients (Cuentas et al., 2017).
The same bacteria has also been tested in hundreds of people with functional constipation, and adding it to the laxative taken by patients significantly improves the effectiveness of the treatment (Li et al., 2012; Liu et al., 2012).
