Prostate Problems: Causes and Potential Approaches
Benign prostatic hyperplasia (BPH) is the most common, and prostate cancer is the most dangerous, problem with the prostate gland. Although the causes of them are still not well understood, the mechanisms behind their occurrence are becoming increasingly clear from science, and based on this, new and new gentle methods of management and prevention are being studied.
Pumpkin seed extract
Pumpkin seed extract acts as an inhibitor of the enzyme 5-alpha reductase, which is responsible for converting the hormone testosterone into the more potent androgen dihydrotestosterone. This is a sought-after effect in some androgen-dependent conditions, including benign prostatic hyperplasia, prostate cancer, and hair loss (androgenic alopecia).
One of the first studies to clearly demonstrate the potential of pumpkin seeds was conducted among 2245 patients with BPH over 12 weeks. It reported a 41.4% improvement in symptoms of the condition (on the I-PSS scale) and a 46.1% improvement in quality of life (Friederich et al., 2000).
These findings were confirmed by a subsequent one-year study conducted in Germany among 1431 men. It found that the administration of pumpkin seed extract led to a clinically significant decrease in the I-PSS index with excellent patient tolerance (Vahlensieck et al., 2015). Another team of authors found that a combination product with pumpkin seed oil had a positive effect on sexual and reproductive function (Hashemi, 2013).
In addition to its therapeutic potential, pumpkin seed oil may have a preventive role against benign prostatic hyperplasia (Gossell-Williams et al., 2006).
Comparisons between the natural extract and the drug tamulosine confirmed the effectiveness of pumpkin seed and its significantly better tolerability. Adverse effects were not recorded in any of the patients in the plant extract group and in 15% of the patients taking the drug (Zerafatjou et al., 2021).
The combination of pumpkin seed and saw palmetto extracts that underlie Proxel Duo has also been well-tested and confirmed as a “safe and highly recommended option for men with symptomatic BPH” (Carbin, Larsson & Lindahl, 1990; Marks et al., 2000; Coulson et al., 2013).
Saw palmetto (Serenoa repens)
Saw palmetto extract is the most popular phytotherapeutic agent used in the management of benign prostatic hyperplasia and its associated urinary symptoms such as frequent and urgent urination, incontinence, nocturia, etc. It is extracted from the fruits of the American saw palmetto (Serenoa repens, Sabal serrulata), whose use in various prostate gland conditions was documented more than 100 years ago in the 1880s.
Saw palmetto extract has a similar effect on the enzyme 5-alpha reductase as pumpkin seed extract (Di Silverio et al., 1992; Iehle et al., 1995; Bayne et al., 1997). It also has pronounced antiestrogenic (Paubert-Braquet et al., 1995) and anti-inflammatory properties (Paubert-Braquet et al., 1997). It has also been shown to inhibit cell proliferation under the influence of prolactin and growth factors (Vacher et al., 1995; Paubert-Braquet et al., 1997), which is one of the mechanisms behind prostate cancer.
In recent decades, a number of clinical and observational studies have been conducted among patients with benign prostatic hyperplasia. The majority of them confirm that saw palmetto contributes to significant relief of urinary symptoms such as nocturia, difficult and weak stream, incomplete bladder emptying, etc. (Champault et al., 1984; Roveda & Colombo, 1994; Bach & Ebeling, 1996; Braeckman et al., 1997; Stepanov et al., 1999; Boyle et al., 2000; Gerber et al., 2001; Tacklind et al., 2012; Vela-Navarrete et al., 2018; Alcaraz et al., 2021)
Some clinical trials have even compared saw palmetto with established pharmaceutical 5-alpha reductase inhibitors such as finasteride and dutasteride. The natural extract has been shown to have similar efficacy to the drugs, but is also significantly better tolerated and, in addition, improves sexual function (Carraro et al., 1996; Sokeland, 2000; Suter et al., 2012; Tacklind et al., 2012).
It has also been found that supplementing standard tamsulosin treatment with saw palmetto extract reduces the risk of decreased libido (Alcaraz et al., 2022). Other studies have found that the tolerability of saw palmetto is superior to that of tamsulosin (Alcaraz et al., 2021).
Turmeric
The active substance in turmeric extract – curcumin – has a number of unique properties, including anti-inflammatory, antioxidant, immunomodulatory and antitumor activity (Moghadamtousi et al., 2014; Momtazi et al., 2016; Bagherniya et al., 2018; Mollazadeh et al., 2019; Hasanzadeh et al., 2020).
Based on information from various studies, it can be concluded that curcumin has a pronounced anticancer effect (Aggarwal, Kumar & Bharti, 2003; Agrawal & Mishra, 2010; Momtazi & Sahebkar, 2016; Teymouri et al., 2017; Mohajeri et al., 2020) and several preclinical studies have already shown that the ingredient favorably regulates gene expression in prostate cancer cells (Mukhopadhyay et al., 2001; Nakamura et al., 2002; Chendil et al., 2004; Boccellino et al., 2022).
Current evidence from 11 studies on 745 participants with various prostate conditions shows that taking turmeric extract contributes to regulating prostate-specific antigen (PSA) levels, improving quality of life, oxidative stress markers, the feeling of incomplete bladder emptying, urinary frequency, urgency, weak stream, difficulty urinating, and nighttime awakening, without significant adverse effects among patients (Bagherniya et al., 2021).
Moreover, curcumin has also demonstrated preventive potential with regard to prostate cancer, interacting with a number of the mechanisms behind the development of the disease and preventing the multiplication and metastasis of prostate tumor cells (Teiten et al., 2010; Termini et al., 2020).
Zinc
Zinc is an essential dietary mineral that performs numerous roles in the human body - it catalyzes a number of reactions, plays the role of a structural component in hundreds of metalloproteins, and contributes to normal testosterone levels, normal male sexual and reproductive function.
It is most concentrated in the prostate gland, where it contributes to its function and metabolism, and its normal levels there are one of the hallmarks of healthy prostate cells (Franz et al., 2013; Karunasinghe, 2022). In patients with prostate cancer and BPH, on the other hand, 83% and 61% lower tissue zinc levels are observed, respectively (Christudoss et al., 2011).
The hypothesis of zinc deficiency as a direct cause of prostate cancer is even being discussed in scientific circles (Costello & Franklin, 2011) and it is believed that correcting it may be a promising approach in the prevention and treatment of prostate cancer (Costello & Franklin, 2011; Zhang et al., 2023).
This is why a number of authors recommend zinc supplementation for men over middle age, who have an increased risk of prostate cancer and BPH in combination with a high incidence of suboptimal zinc status due to the body's reduced ability to absorb the mineral, underlying diseases, an insufficiently varied diet, and certain medications (Sauer et al., 2020).
