A nutraceutical approach to male sexual health
Erectile dysfunction is a common problem, with current estimates currently affecting about 1/3 of men overall (Gerbild et al., 2018) and half of men over 40, with the risk increasing significantly after this age (Feldman et al., 1994).
More worryingly, 26% of men seeking medical help for erectile dysfunction are younger (Capogrosso et al., 2013). Given the sensitive nature of the condition and the reluctance of many men to talk about it, experts estimate that the actual proportion of those affected is significantly higher.
This means that a significant percentage of these men do not have access to conventional erectile dysfunction medications, as they are available only by prescription. Another percentage avoids their use because they do not always provide satisfactory results (Friedman & Friedman, 2015) and carry a risk of side effects such as nasal congestion, facial flushing, headaches, visual disturbances, lower back and neck pain, dyspepsia, abdominal pain, metabolic disorders, and in rare cases, serious events such as heart attack and stroke (Ückert, Kuczyk & Oelke, 2013).
Furthermore, they do not affect libido, the lack of which often accompanies or causes erection problems and is one of the main components of sexual dysfunction in men.
Current scientific evidence suggests that many men can overcome sexual dysfunction without relying on medication, but by focusing on lifestyle changes. A study published in the Journal of Sexual Medicine (Martin et al., 2014) found that remission in one in three men was due to such changes.
These include dietary improvements that address a number of risk factors for erectile dysfunction (poor markers of cardiovascular and metabolic health, sleep apnea, and other sleep problems) and provide an abundance of foods and bioactive substances with proven effects on male sexual function.
Saffron
Saffron is an exotic spice, natural dye, and herb, consisting of the dried stigma of the saffron crocus Crocus sativus. Its use dates back at least 3,000 years and is traditional in countries such as India, Greece, France, Spain, Italy, and Iran.
The role of saffron crocus in male sexual performance has been investigated in a number of clinical studies. As early as 2009, its extract was tested in 20 men with erectile dysfunction, with just 10 days of intake being enough to increase sexual function, the number and duration of erections (Shamsa et al., 2009).
Another study in men with major depressive disorder showed that daily intake of saffron crocus extract significantly improved erectile function and sexual satisfaction compared to placebo (Modabbernia et al., 2012).
A saffron extract formula has also been tested in men with benign prostatic hyperplasia and erectile dysfunction. After three months of treatment, patients' quality of life improved significantly, with improvements reported in various parameters of sexual function and sex life (Cai et al., 2013).
Clinical studies on the properties of saffron have been analyzed in two recent systematic reviews, one focusing on erectile function and the other on sexual function in general. Both reviews provide positive conclusions regarding the beneficial effects of the ingredient on erections, libido, and sexual satisfaction (Maleki-Saghooni et al., 2018; Ranjbar & Ashrafizaveh, 2019).
Apple and grape polyphenols
Apple fruit and grape seeds are rich in polyphenols with potent antioxidant and vasodilating (relaxing and widening blood vessels) properties. Thus, they can directly affect erectile dysfunction, which is often due to impaired vascular function and vasodilation in the penis.
It is believed that the vasodilating effect of these polyphenols is due to increased activation of the enzyme nitric oxide synthase (eNOS) in the body. The same enzyme also synthesizes nitric oxide (NO), which is essential for erection.
This hypothesis was confirmed by a clinical trial in 94 men over 45 years of age with declining erectile function (Allaert et al., 2021). After 28 days of taking the combination of polyphenols in combination with saffron extract, a 22% increase in the Erection Hardness Scale (EHS) was observed in 73.8% of the participants, who reported the following positive impressions:
· 71% would recommend the combination as a reliable means of combating erectile dysfunction.
· 66% are very satisfied with using the formula and have noticeably better erections.
· 58% are more confident about future sexual acts, relying on this combination.
Fenugreek
Fenugreek (Trigonella foenum-graecum) is a clover-like aromatic herb native to the Mediterranean region, Southeast Europe, and Western Asia. Known in our country primarily for its culinary uses, its seeds are rich in saponin glycosides with potent biological properties.
It is to them that the power of the patented Testofen® extract of the American company Gencor, which is part of Maxistim XL, is due. It is standardized for 50% content of a unique matrix of saponin glycosides and is the best clinically studied fenugreek extract in the world. This makes it a key ingredient in some of the most effective formulas for improving sports performance and optimizing men's health. The tests only confirm what thousands of users around the world report as a result of its use.
A 2016 clinical study showed that 12 weeks of Testofen® increased sexual function by 15%, with improved indicators such as sexual arousal and desire, as well as a 2- to 3-fold increase in morning erections and sexual intercourse (Rao et al., 2016).
Another clinical trial in healthy men demonstrated that a 6-week protocol with the same extract in combination with zinc positively affected sexual behavior and cognition, arousal, and orgasm. At the end of the 8-week study period, the fenugreek extract group reported more strength, greater muscle mass gain, greater fat loss, and higher testosterone levels (Rao, Mallard & Grant, 2020).
A recent scientific review of the available evidence concludes that fenugreek extract has a significant effect on testosterone levels in men, reinforcing its potential role in sports nutrition and a wide range of adverse “male” conditions such as andropause, sexual dysfunction, and related mental health disorders (Mansoori et al., 2020).
Zinc
Zinc is an essential dietary mineral that performs numerous roles in the human body – it catalyzes a number of reactions and plays the role of a structural component in hundreds of metalloproteins.
It has long been known that zinc levels in the body are directly related to testosterone levels, and increased urinary zinc excretion poses a risk of lowering the male sex hormone (Prasad et al., 1996; Chang et al., 2011; Zeng et al., 2013). This is important because testosterone is deeply involved in every phase of the male sexual response, and low levels are one of the factors for sexual dysfunction in men (Rastrelli, Corona & Maggi, 2018).
Clinical studies on zinc-deficient men found that oral zinc supplementation for 6 weeks increased testosterone levels by 84% (Jalali et al., 2010). Adding zinc to the diet of infertile men with low testosterone levels significantly increased sex hormone levels, with 25% of couples conceiving within 3 months of the trial (Netter, Nahoul & Hartoma, 1981).
Zinc supplementation in athletes with normal testosterone levels has been shown to increase testosterone levels immediately after exercise (Neek, Gaeini & Choobineh, 2011). This is important because physical exertion can lead to a decline in testosterone levels. This effect has been confirmed in both non-athletic men and elite athletes after strenuous exercise – in both scenarios, zinc supplementation preserved sex hormone status (Kilic, 2007; Kilic et al., 2006).
The zinc in the Maxistim XL formula is in the form of the patented organic compound monomethionine (L-OptiZinc® ZML-200). The amino acid methionine has been added to it, which increases the efficacy of the valuable mineral. It has been found that zinc monomethionine has significantly better bioavailability, which is a prerequisite for significantly better antioxidant and immunostimulating properties (Chien et al., 2006).
Tribulus terrestris
Tribulus terrestris is an herb used in traditional Ayurvedic medicine and better known in our country as "grandmother's teeth". It has long been used as a means of improving men's health, and its benefits shared by word of mouth have brought it mythical fame among strength and sex athletes from yesterday to today.
It turns out that this fame is not without its flaws. Although its halo as a sports supplement has not been scientifically validated, there is convincing evidence for its properties on the sexual front. In 2005, it was found that taking tribulus extract stimulates male sex hormone receptors in the brain and increases the number of a special class of neurons involved in the production of nitric oxide (Gauthaman & Adaikan, 2005). This finding reveals a plausible biological mechanism for the herb's positive effect on both libido and erection.
One of the first studies to test the clinical effects of taking fenugreek was conducted on 63 men with oligozoospermia (low sperm count). After 12 months of taking it, not only was there an improvement in sperm characteristics, but also a significant increase in erectile function and frequency of intercourse (Sellandi, Thakar & Baghel, 2012).
In men with partial androgen deficiency, erectile dysfunction, and low libido, taking tribulus extract for 3 months led to increased testosterone levels and erectile function, measured by the International Index of Erectile Function-5 (IIEF-5) (Roaiah et al., 2016).
In a subsequent experiment, 180 men with erectile dysfunction took either an extract of elderberry or a placebo for 12 months. Significant improvement in potency in those taking the herb began to be observed after the fourth week, and at the end of the experiment, nearly three times better sexual function, as measured by IIEF-5, was reported. Improvements were also reported in terms of sexual satisfaction and orgasmic function (Kamenov et al., 2017).
Tribulus terrestris’s effects are likely due to its potent adaptogenic properties. Several experiments have found that the herb extract has comparable effects to some antidepressants, with its intake completely normalizing stress hormone levels in subjects subjected to chronic stress (Deole et al., 2011; Wang et al., 2013).
Maca
Maca (Lepidium meyenii) is a member of the Cruciferous family and is cultivated for its bioactive compound-rich root, which was traditionally used by the Incas as a means of increasing physical endurance and sexual potency.
One of the first controlled trials to test these effects in healthy adult men was conducted in 2002. The results showed a significant increase in libido after eight weeks of daily intake of maca extract (Gonzales et al., 2002).
Another 2009 study measured changes in endurance and libido in a group of athletes after 2 weeks of taking a poppy root extract. Although no improvement in endurance was recorded, all participants reported a greatly increased sexual appetite (Stone et al., 2009).
The observed effect has also been validated in patients with post-SSRI sexual dysfunction – reduced sensitivity, erectile function and libido as a result of antidepressant treatment. Taking maca for 12 weeks helped participants to significantly improve their sexual performance, with the effect being dose-dependent – proportional to the amount of extract taken (Dording et al., 2008).
Dry maca extract is also effective in men with diagnosed erectile dysfunction, with significant improvements in erectile function and sexual well-being, measured according to the International Index of Erectile Function (IIEF-5) and Satisfaction Profile (SAT-P), reported after a 12-week course of intake (Zenico et al., 2009).
Muira puama
Muira puama (Ptychopetalum olacoides) is a shrub native to the Amazon and used in traditional Brazilian medicine as a stimulant – of the nervous system, appetite, and, of course, sexual desire and performance. It is administered by local healers orally as a decoction and topically as a paste of crushed leaves.
The first animal tests of the herb validated its potential to increase potency due to its ability to cause relaxation of the muscles of the corpus cavernosum in the penis (Antunes et al., 2001).
Clinical trials in middle-aged men with erectile dysfunction showed that taking a formula with muira puama for three months led to improved erections and sexual satisfaction (Nguyen, Rajfer & Shaheen, 2018).
Other studies have found that the extract contributes to libido stimulation, leading to significantly increased sexual appetite and frequency of intercourse (Waynberg & Brewer, 2000).
L-arginine
L-arginine is an amino acid that serves as a direct biological source of nitric oxide – the molecule that, under the influence of sexual arousal, leads to the dilation of blood vessels in the penis and the subsequent erection (Leung, Christ & Melman, 2004).
Recently, it was found that men with erectile dysfunction have significantly lower levels of L-arginine (Barassi et al., 2017), further fueling its research as a potential therapeutic solution and alternative to prescription medications.
A significant amount of research on L-arginine has accumulated in a short time, and an overview of their results can be found in a meta-analysis published in 2019 in The Journal of Sex Medicine (Rhim et al., 2019). Based on data from 10 randomized controlled trials with a total of 540 participants, it was found that L-arginine supplementation significantly affects erectile function, increases sexual satisfaction, and improves orgasmic function.
L-carnitine
L-carnitine is an amino acid-derived compound that is naturally present in the human body. It is produced in the brain, liver, and kidneys from the amino acids lysine and methionine, but its needs are met mostly from external dietary sources such as red meat, dairy products, and fish.
A large body of clinical data demonstrates that L-carnitine supplementation increases sperm count and motility in men with infertility due to various causes (Costa et al., 1994; Lenzi et al., 2003; Lenzi et al., 2004; Cavallini et al., 2004; Balercia et al., 2005; Zhou, Liu & Zhai, 2007; Cheng & Chen, 2008; Eidi et al., 2009; Wang et al., 2010; Moradi et al., 2010; Peivandi, Karimpour & Moslemizadeh; 2010).
A large-scale meta-analysis from 2020, covering all clinical studies on the topic, concluded that taking carnitines increases the chance of pregnancy 3.7 times compared to placebo (Zhang et al., 2020).
It has also been found that supplemental intake of the substance by men with abacterial prostatovesiculoepididymitis (inflammation of the prostate, seminal vesicles, and epididymis) contributes to increasing sperm count and motility (Vicari, La Vignera & Calogero, 2002).
Other studies have investigated the effects of various L-carnitine combination formulas. A clinical trial in men with asthenozoospermia found that combining L-carnitine with L-arginine, zinc, and Panax ginseng for 3 months increased sperm motility and sexual health (Morgante et al., 2010). The same combination was successfully administered two years later in men with chronic prostatitis, contributing to increased sperm concentration and motility (Cai et al., 2012).
L-carnitine is also included in Proxide Plus, which was tested by a team of Serbian scientists in 2019 in men with oligoasthenozoospermia. Taken twice daily for 6 months, it led to an increase in sperm volume, motility, and vitality, as well as a decrease in sperm DNA fragmentation (Micic et al., 2019).
The combination of acetyl-L-carnitine and propionyl L-carnitine, characteristic of the Maxistim XL formula, has been tested for alleviating the symptoms of the so-called "male menopause" - low testosterone levels associated with advancing age. Taking both forms of carnitine for 6 months has been shown to be effective in improving sexual performance, depression and fatigue in older men. What is impressive is that in this trial the combination of carnitines has an effect comparable to the effect of direct administration of testosterone (Cavallini et al., 2004).
The same tandem of carnitines was also used to restore sexual function in 96 men with prostatectomy (surgical removal of the prostate, which often leads to erectile dysfunction). It was found that the combination of acetyl-L-carnitine and propionyl L-carnitine significantly improved the effectiveness of the main treatment and were safe and reliable concomitant agents for restoring potency (Cavallini et al., 2004).
Vitamin B3
Vitamin B3 (niacin) is a popular heart health ingredient due to its researched ability to dilate blood vessels, lower triglycerides, and increase “good” cholesterol (HDL). However, these properties also appear to be beneficial for sexual health, as adding niacin to the diet of patients with low HDL levels has been found to improve blood circulation and vascular function (Figueiredo et al., 2014).
Improved blood flow to peripheral tissues in men can contribute to improving many aspects of sexual function, including achieving and maintaining an erection. Managing lipid profiles can also help combat atherosclerosis, which is one of the leading causes of erectile dysfunction.
These assumptions were tested in a clinical study with 160 men, in which vitamin B3 supplementation for 12 weeks improved erectile function scores among patients with moderate to severe erectile dysfunction (Ng et al., 2011).
A subsequent study examined the effects of a combination of L-carnitine, L-arginine, and niacin in 54 men with erectile dysfunction. At the third month of the study, significant improvement in erectile function was found in 40% of cases and partial improvement in 77% of participants (Gianfrilli et al., 2012).
