Dysmenorrhea, or painful menstruation, is defined as cramping in the lower abdomen before or during menstruation. Usually the pain is mild. But for some women, menstrual cramps can be so severe that they interfere with daily activities for several days a month. The painful cycle is caused by hormone-like substances called prostaglandins, which are produced in the lining of the uterus. They cause the muscles and blood vessels of the uterus to contract during menstruation. In the first two days of menstruation, the level of prostaglandins is the highest and then the pain is the strongest. In the following days, their level decreases, and so does the pain, as the bleeding continues and the lining of the uterus is shed. In some women, severe menstrual pain is accompanied by other symptoms - diarrhea, nausea, vomiting, headache and dizziness, which are also typical side effects of prostaglandins. Menstrual pain can be classified as: Primary dysmenorrhea - painful menstruation that is not related to any pelvic disease. Primary dysmenorrhea begins soon after girls start menstruating. For many women with primary dysmenorrhea, periods become less painful as they age. This type of menstrual pain also usually subsides after childbirth. Secondary dysmenorrhea – pain during menstruation that is caused by some disease, e.g. endometriosis, fibroids, ovarian cysts, pelvic inflammatory disease, etc. The standard approach to treating dysmenorrhea includes nonsteroidal anti-inflammatory drugs (NSAIDs) and hormonal contraception, both of which help suppress the production of prostaglandins and thus reduce pain. Patients who do not tolerate, have contraindications for taking NSAIDs or wish to avoid them, can try to replace them with an herbal antispasmodic and inflammatory modulator. Nutrients and plant extracts with a role in menstrual pain relief Magnesium – studies with magnesium have found a reduction in menstrual pain and a decrease in prostaglandins in the blood.
