But this complex structure can sometimes be damaged, so that messages on it do not reach their recipients or reach them in a corrupted form. This often leads to suffering and pain. Spine and Peripheral Nerves You can imagine the spine as a kind of highway of the central nervous system, through which signals from the brain are transmitted to the most distant corners of the human body. The nerves supplying all these nooks and crannies branch off the entire length of the spine. Exactly 31 pairs of spinal nerves originate from the spinal cord, starting from the so-called nuclei (clusters of neurons) in it, passing through the intervertebral foramina and branching into numerous smaller peripheral neurons that supply different parts of the body. For example, neurons that come out of the lower spine branch all the way to the toes. Peripheral nerves make up the peripheral nervous system and include both motor and sensory neurons: Sensory neurons are able to perceive sensory stimuli, or in other words, they allow us to feel things like cold, heat, and pain. These neurons are made up of fibers called sensory fibers, which have their own classification. Mechanoreceptor fibers are responsible for sensing body movements and pressure on the body, while nociceptor fibers are responsible for sensing injury. Motor neurons reach the muscles and produce muscle contractions, and hence movement. They are made up of motor fibers. Neuropathy - what is it? Broadly speaking, neuropathy is nerve damage that can affect both the inner and outer structure of a neuron. The axon (so-called nerve fiber) is the inner part of the nerve cell that is responsible for conducting nerve impulses. If we compare the neuron to an electrical cable, the axon is the wire of that cable. The myelin sheath is the fatty covering of the neuron, which has a protective and assisting effect in the transmission of information along the nervous system. Going back to the electrical cable analogy, it is its insulation. How do nerves cause pain? A number of factors such as trauma, illness, intoxication, aging, etc. may lead to irritation or damage to the neurons of the peripheral nervous system. For example, the spinal vertebrae and intervertebral discs are located in close proximity to neurons exiting the spinal cord, which creates multiple conditions for compression of these neurons and the generation of pain in the lower back, back, and neck. A typical example is a disc herniation, in which there is displacement of the intervertebral disc (protrusion), compression of a nearby nerve, and pain not only in the area of compression, but sometimes along the entire length of the neuron. In one study of 212 patients with peripheral neuropathy, a combination of uridine, folic acid, and vitamin B12 significantly reduced pain intensity and the number of painful sites. Another study in 48 people with carpal tunnel syndrome confirmed the effectiveness of this combination, finding 40% less pain after 2 months of use. The triad of vitamin B12, uridine and cytidine effectively reduced neural pain in 400 patients with compressive neuralgia (pain from a pinched nerve) – significantly more than vitamin B12 alone. Uridine has also been shown to significantly improve the condition of patients with diabetic neuropathy (nerve damage resulting from high blood sugar). Magnesium Magnesium regulates various processes in the human body, one of which is neuromuscular conduction. Studies show that the average dietary intake of magnesium is relatively low, reaching only 30 to 50% of the recommended daily intake, which is 420 mg for men and 320 mg for women per day. A clinical study conducted with patients with chronic low back pain showed a significant reduction in pain and improved mobility in patients taking magnesium. Vitamin D Vitamin D is a hormone-like vitamin, low levels of which are often associated with chronic pain problems (Shipton, EE, & Shipton, EA, 2015) and higher pain medication use by pain patients (Bergman, P., et al ., 2015). Conversely, a number of studies have found that supplemental intake of the vitamin contributes to the reduction of chronic pain in cancer patients (Björkhem-Bergman, L., & Bergman, P., 2016), musculoskeletal problems (Gendelman, O., et al., 2015), fibromyalgia (Wepner F., et al., 2014) and diabetic neuropathy (Lee, P., & Chen, R., 2008). The physiological mechanisms linking vitamin D and pain are not fully understood, but evidence suggests that its insufficient levels affect the function of peripheral nerves and the parasympathetic division of the nervous system (Helde-Frankling, M., & Björkhem-Bergman, L., 2017 ). The presence of vitamin D receptors in the central nervous system and its effect on a number of neurotransmitters is probably also relevant, as is its pronounced anti-inflammatory function. Cytidine Monophosphate Cytidine is an important component of RNA with a key role in intercellular signaling and neural conduction. There is a significant body of preclinical and clinical data supporting its effectiveness against neuropathic pain, particularly when administered in combination with uridine and vitamin B12. Several studies have demonstrated its beneficial effects in the recovery from central nervous system trauma, as well as in the recovery of motor function after peripheral nerve damage. Taking cytidine has been shown to help relieve neuropathic pain and restore movement just 4 weeks after sciatic nerve injury (Wibowo, S., & Meliala, L., 2016). In another trial, a three-month course of treatment with uridine + cytidine was sufficient to significantly improve sensation and reduce pain in 75 patients with diabetic neuropathy (Ndiaye, M, et al., 2015). A 10-day administration of a combination of cytidine, uridine, and vitamin B12 effectively reduced pain and improved function in 41 patients with acute nontraumatic low back, pelvic, and neck pain (Mibielli, MA, et al., 2010). Why should you act as soon as possible if you are in pain? If you suffer from neuropathy or frequent numbness of the limbs, early conservative intervention to restore normal function of the nervous system is especially important. Its delay may necessitate resorting to more drastic and invasive treatment. It is believed that in a number of cases, even pain of other origin can turn into neuropathic if it is not addressed in time. Prolonged time spent in a state of pain can lead to changes in the central nervous system that heighten pain sensitivity and turn even light touch into a painful experience. In science, this phenomenon is known as "central sensitization". As with other types of chronic pain, delayed treatment for neuropathic pain can increase the risk of other health problems. Depression, anxiety, sleep disorders, and inability to work and engage in other important activities are just some of the complications associated with neglected neuropathy.
