Understand neuropathic pain to manage it

Pain is an inevitable but unpleasant part of our biology. Common causes of it are diseases affecting the nervous system:

  • Sciatica
  • Lumbago
  • Discopathy
  • Cervical radiculitis
  • Diabetic neuropathy
  • Intercostal neuralgia
  • Plexiglass

The pain associated with them can be characterized as neuropathic or neuropathic. Sometimes it comes in the form of a sudden burning stab, often in the middle of the night. Other times it can be accompanied by a chronic numbness, tingling, tickling, or burning that simply refuses to go away.

To better understand neuropathic pain, we need to talk about the basic purpose of pain in general. Pain serves as a kind of alarm system for the human body. When our hand finds itself dangerously close to a flame, our nerves send a pain signal to the brain: “This person is about to get burned!” In response, our brain initiates a withdrawal reflex before we can even get burned.

Nerve damage can damage our pain alarm system. This can lead to numbness of parts of the body – the absence of pain signals when they are needed. And even more worryingly – hypersensitivity. Damaged nerves can sound a “false alarm” and cause us to experience very real pain that is not reciprocal to the injury or for which there is no external cause.

In many cases of neuropathic pain, quite normal positions and movements of the body (such as standing or walking) become painful. Some patients may begin to feel pain from the gentle touch of their own clothes or from the sheets that until recently provided them with comfort and warmth during sleep. This condition is known in the medical literature as “allodynia.” Left to their own devices, neural damage can worsen over time and “lock” us in a permanent state of pain.

Fortunately , there are ways to control and even treat neuropathic pain . This is because nerves are metabolically active tissue with their own vascular system ( vasa nervorum ) that supplies them with nutrients that are crucial to their health. Such substances include:

B-vitamins

Deficiency of B vitamins leads to neuropathies, degeneration, sensory impairment, and impaired recovery of neural pathways. According to statistics, about 32% of people with B vitamin deficiency develop generalized peripheral nerve damage. Conversely, their intake can contribute to the control of pain, the regeneration of the nervous system, and the normalization of reflexes and sensitivity. It is no coincidence that they are called “neurotropic”.

Uridine monophosphate

It is involved in the creation of RNA and a number of neuronal structures such as dendrites (neuronal growths) and myelin sheath (neuronal insulation). Taking uridine monophosphate accelerates the “repair” of damaged nerves and can suppress pain signals.

In one study of 212 patients with peripheral neuropathy, a combination of uridine, folic acid, and vitamin B12 significantly reduced pain intensity and the number of painful areas. Another study in 48 people with carpal tunnel syndrome confirmed the effectiveness of this combination, finding a 40% reduction in pain after 2 months of treatment.

The triad of vitamin B12, uridine and cytidine effectively reduced neuralgia pain in 400 patients with compressive neuralgia (pain from a pinched nerve) – significantly more than vitamin B12 alone. Uridine has also been shown to significantly improve the condition of patients with diabetic neuropathy (nerve damage caused by high blood sugar).

Magnesium

Magnesium regulates various processes in the human body, one of which is neuromuscular conduction. Studies show that the average dietary intake of magnesium is relatively low, reaching only 30 to 50% of the recommended daily intake, which is 420 mg for men and 320 mg for women per day. A clinical study conducted with patients with chronic low back pain showed a significant reduction in pain and improvement in mobility in patients taking magnesium.

Vitamin D

Vitamin D is a hormone-like vitamin, low levels of which are often associated with chronic pain problems (Shipton, EE, & Shipton, EA, 2015) and higher intake of painkillers by patients with pain (Bergman, P., et al., 2015).

Conversely, a number of studies have found that supplemental vitamin intake contributes to the reduction of chronic pain in patients with cancer (Björkhem-Bergman, L., & Bergman, P., 2016), musculoskeletal problems (Gendelman, O., et al., 2015), fibromyalgia (Wepner F., et al., 2014), and diabetic neuropathy (Lee, P., & Chen, R., 2008).

The physiological mechanisms linking vitamin D and pain are not fully understood, but evidence suggests that insufficient levels affect peripheral nerve function and the parasympathetic division of the nervous system (Helde-Frankling, M., & Björkhem-Bergman, L., 2017).

The presence of vitamin D receptors in the central nervous system and its effect on a number of neurotransmitters is probably also important, as is its pronounced anti-inflammatory function.

Cytidine monophosphate

Cytidine is an important component of RNA with a key role in intercellular signaling and neural conduction. There is a significant amount of preclinical and clinical data confirming its effectiveness against neuropathic pain, especially when administered in combination with uridine and vitamin B12.

Several studies have demonstrated its beneficial effects in the recovery from central nervous system trauma, as well as in the restoration of motor function after peripheral nerve injury. Cytidine supplementation has been shown to help relieve neuropathic pain and restore movement just 4 weeks after sciatic nerve injury (Wibowo, S., & Meliala, L., 2016).

In another trial, a three-month course of uridine + cytidine treatment was sufficient to significantly improve sensation and reduce pain in 75 patients with diabetic neuropathy (Ndiaye, M, et al., 2015). A 10-day combination of cytidine, uridine, and vitamin B12 effectively reduced pain and improved function in 41 patients with acute non-traumatic low back, pelvic, and neck pain (Mibielli, MA, et al., 2010).

URIMIL GEL / URIMIL GEL is part of the Urimil series, developed with care for the peripheral nervous system. The gel is formulated with five active ingredients with a warming and relieving effect. Uridine monophosphate (UMP). Topically applied uridine is absorbed through the skin, leading to the restoration of nerve structure and function (Hartinger et. al., 2012). Uridine is involved in the synthesis of RNA, proteins and phospholipids, which are key for the construction of cell membranes and nerve cell sheaths. It stimulates the reproduction of nerve cells and the formation of new nerve growths (Manhães et. al., 2017). Pharmacological studies indicate that uridine accelerates and supports neuromuscular recovery and suppresses pain conduction in the spinal cord.

Arnica (Arnica montana) is a plant of the sunflower family, which grows mainly in Siberia, Central Europe, Canada and South America. The main active ingredients in arnica extract are the so-called sesquiterpene lactones. The most important of them is perhaps helenalin , which has anti-inflammatory (Lyss et. al., 1998), cooling and analgesic effects. Applied topically, arnica extract is used to treat osteoarthritis, rheumatoid arthritis, relieve muscle pain, and heal bruises, sprains and wounds.

Devil's claw (Harpagophytum procumbens) is a perennial plant that can only be found in South Africa and is therefore also called African geranium. Its active ingredients in its extract have a strong anti-inflammatory and analgesic effect and its topical application leads to improved mobility and quality of life in patients with osteoarthritis, rheumatoid arthritis, tendonitis and low back pain (Wegener, 2000; Denner, 2007).

Methylsulfonylmethane (MSM) is an organic sulfur compound, a natural source of sulfur for the body. MSM has a high ability to pass through cell membranes and affects various cells and tissues. Applied topically, it smoothes and softens the skin, increases energy, circulation and metabolism, and accelerates wound healing. It has antioxidant and anti-inflammatory effects (Butawan et. al., 2017), which play an important role in patients with osteoarthritis. MSM is effective in muscle pain after physical exertion due to the delivery of sulfur to the connective tissue and the prevention of local inflammation as a result of microtrauma.

Magnesium (Mg) relaxes muscles and reduces pain by blocking NMDA receptors in nerve cells. When applied topically, it passes through the skin and increases magnesium levels in the body, with these levels depending on the concentration of the applied preparation and the time of exposure (Kass et. al., 2017; Chandrasekaran et. al., 2016). It is a cofactor in over 300 enzymatic reactions in the body and is important for muscle contraction, nerve signaling and neuromuscular conduction. Therefore, magnesium is important in the treatment of diseases of the peripheral nervous system and the musculoskeletal system, significantly alleviating symptoms such as pain, numbness and tingling (Na et. al., 2011).

Method of application:

Rub Urimil gel with gentle massage movements into the affected area until completely absorbed by the skin. Apply to clean skin and for maximum effect do not wet the area for at least 3 hours after application. The speed of the effect may vary from person to person. Apply twice daily.
