Bedwetting in Children: Possible Causes and Potential Approaches
Bedwetting in children (nocturnal enuresis) is the involuntary loss of urine that occurs only at night. It is a completely normal release of the bladder, with the peculiarity that it occurs at an inappropriate and socially unacceptable time and place. This creates significant stress for both the child and his family.
The WHO classifies bedwetting as a mental and emotional disorder that occurs at least twice a month in children under the age of seven and at least once a month in children seven years of age or older.
The causes of bedwetting are not fully understood, but available evidence suggests an interaction between a range of genetic, physiological, and psychological factors.
It is known that about 70% of children with nocturnal enuresis have a relative who has suffered from the same condition. Statistics show (Traisman, 2015) that the risk of bedwetting for a child is:
· 44% if only one parent had nocturnal enuresis and
· 77% if they had both.
The main physiological factors for bedwetting in children include:
Nocturnal polyuria, or the production of more urine at night. This phenomenon is due to a disruption in the circadian rhythm of the antidiuretic hormone vasopressin, which is observed in about 70% of children with bedwetting (Rittig et al. 1989). Under normal conditions, the secretion of this hormone increases at night, resulting in reduced urine production.
Reduced nocturnal bladder capacity: Children with nocturnal enuresis may have a normal bladder volume but a reduced urine volume, in which the bladder muscles contract and cause an urge to urinate. A recent Chinese study of over 1,500 children with nocturnal enuresis found that about 40% of them had reduced functional bladder capacity at night (Lui et al., 2008).
Impaired sleep quality and architecture: The so-called reduced arousal and inability to wake up despite the urge to urinate is a relatively poorly understood factor in the pathophysiology of bedwetting. Many parents of children with nocturnal enuresis comment that their child sleeps too deeply. A 1999 study by Hunsballe found that disturbances in sleep architecture, expressed in longer periods of deep sleep, were indeed associated with a higher risk of nocturnal enuresis.
Psychological factors are rarely the cause of bedwetting, but they can be its consequence. Stressful events such as parental separation, a death or birth in the family, starting kindergarten or school can contribute to so-called intractable symptomatic enuresis, in which there is wetting during the day as well.
Given the complex causes of nocturnal enuresis, its treatment requires a comprehensive and individualized approach, including behavioral changes in the child. Enuresio Max has been developed as a companion to this approach and is based on three plant extracts that may have a positive effect on the physiological factors for nocturnal enuresis.
Hop extract has well-studied relaxing and nervous system regulating properties. In addition to its use as a spice and preservative in brewing, hops also have a solid scientific record as a sleep-optimizing supplement (Vonderheid-Guth et al., 2000; Dimpfel & Suter, 2008). Its clinical trials have shown not only satisfactory results in terms of sleep quality in people with insomnia (Morin et al., 2005), but also in patients with non-organic sleep disorders (Koetter et al., 2007).
Hops have been shown to increase the activity of the neurotransmitter gaba-aminobutyric acid (GABA), which has a depressant effect on the central nervous system (Aoshima et al. 2006). It also affects both serotonin and melatonin: two chemicals involved in the regulation of nighttime sleep (Abourashed et al., 2004) and circadian rhythm (Maldonado et al., 2009; Weeks, 2009).
Hops are rich in a number of bioactive substances, including ingredients directly related to the health and function of the excretory system:
· Tannins (proanthocyanidins) with astringent properties that help the maturation of the bladder;
· Flavonoids with antidiuretic (vasopressin-like) and smooth muscle relaxing properties.
All of these qualities of hops led Gorad and his team to directly test its effects on 50 children with bedwetting. Results published in 2020 showed that 91.7% of children included in the hop extract therapy completely stopped wetting by the end of the 60th day and remained dry for 15 days after stopping the therapy.
Grape seed extract has potent antioxidant activity (Grases et al., 2015) and may be beneficial for the kidneys, which are particularly susceptible to oxidative stress. Studies have shown that administration of grape seed extract protects the kidneys from damage and improves their function by reducing oxidative stress and inflammatory damage (Ashtiyani et a., 2013; Ulusoy et al., 2012; Ozkan et al., 2012). Other data support its therapeutic potential in urinary tract infections (Ranjitha et al., 2014).
Grape seed extract has been found to help drain the body and act as a diuretic, which can contribute to rapid elimination of urine before sleep (Salmankhan, 2013).
Lemon balm extract has well-documented benefits for mental health and sleep quality. Even single doses of lemon balm can have a calming effect on stressed patients (Kennedy, Little & Scholey, 2004). Other studies have shown that a single dose of lemon balm extract reduces anxiety and improves mood and cognitive function (Scholey et al., 2014).
A 2021 meta-analysis confirmed that the use of an extract of the herb in patients with depression and anxiety for 3 to 56 days significantly improved a number of mental health indicators (Ghazizadeh et al., 2021). A clinical trial in people with mild to moderate anxiety disorders and stress showed that a standardized extract of lemon balm alleviated symptoms and improved sleep after just two weeks (Cases et al., 2011).
Another study conducted in Poland found that a combination of lemon balm and hops improved sleep quality and reduced nightmares (Widy-Tyszkiewicz, & Schminda, 1997). The synergistic effect of these two extracts has been studied in other experiments, which showed that taking them twice a day is enough to achieve a significant reduction in anxiety and improve the ability to cope with psychological and emotional stress (Dimpfel, Pischel & Lehnfeld, 2004).
